隨著國家基本公共衛(wèi)生服務(wù)的正式實施，慢性病患者健康管理就是其中一項重要項目。國家明確要求社區(qū)衛(wèi)生服務(wù)機(jī)構(gòu)為轄區(qū)內(nèi)確診的高血壓或2型糖尿病患者每年免費提供4次面對面隨訪服務(wù)和1次較全面的健康檢查服務(wù)。我院為規(guī)范慢病隨訪，引進(jìn)公衛(wèi)隨訪包。通過手機(jī)就能進(jìn)行隨訪，無需紙質(zhì)版。大大節(jié)省了鄉(xiāng)醫(yī)隨訪時間，并且真實性有了很大的提升。

　　With the formal implementation of basic public health services in the country, the health management of chronic disease patients is one of the important projects. The state clearly requires community health service institutions to provide four face-to-face follow-up services and one comprehensive health examination service for patients with hypertension or type 2 diabetes diagnosed in their jurisdiction for free every year. Our hospital has introduced a public health follow-up package to standardize chronic disease follow-up. Follow up can be conducted through mobile phones without the need for a physical copy. Greatly saving the follow-up time of rural doctors and greatly improving the authenticity.

　　培訓(xùn)后，我院公衛(wèi)人員積極學(xué)習(xí)，認(rèn)真思考，并且下村實際操作，學(xué)會操作后幫助鄉(xiāng)醫(yī)解決一些上傳失敗的問題。對于一些鄉(xiāng)醫(yī)操作還有些不熟悉的情況，我院院長積極安排公衛(wèi)人員，下村為鄉(xiāng)醫(yī)講解疑惑并在現(xiàn)場為其實際操作。?

　　After the training, our hospital's public health personnel actively studied, thought seriously, and went to the village to practice. After learning how to operate, they helped the township doctors solve some upload failure problems. For some situations where the township doctors are still unfamiliar with the operation, our hospital director actively arranged public health personnel to go to the village to explain the doubts to the township doctors and provide on-site practical operations.?

　　公衛(wèi)隨訪包的引進(jìn)大大節(jié)省了隨訪時間，提高了工作效率，真實性有了保障，并且是我院推行無紙化辦公的重要一步。鄉(xiāng)醫(yī)在這次不僅進(jìn)行慢病隨訪，還對該村的個人健康檔案進(jìn)行核實和更新。
